Mrs. Tafare

History:

The patient came to Spine Surgery Specialist in Pune Dr. Sangram Rajale with experiencing pain radiating down the legs for six months, which worsened after a sudden movement during travel 12 days ago. The pain increases with movement but improves with medication. She also has a history of neurogenic claudication, with leg pain after walking for 5 minutes. She reports tingling, numbness, and sleep disturbances, as well as a history of wearing special footwear.

Examination Findings:

Examination of the Lower Spine at Top Spine Pain Clinic in Pune Dr. Sangram Rajale :-

  • Tenderness is felt in the lower back (lumbo-sacral region) and the left sacroiliac joint.

  • The patient has difficulty walking on heels and toes.

  • A straight leg raise (SLR) test is positive on the left side, indicating nerve irritation.

MRI Lumbar Spine:

  1. A tear in the outer part of the disc at L4-L5, with disc material pushing out and pressing on the nerve, causing mild nerve impingement.
  2. Similar findings at L5-S1, with disc material pressing on the spinal cord and nerves, affecting both sides.

Operation Performed:

The patient underwent a left-sided L5-S1 microdiscectomy on 10/09/2024 under general anesthesia, performed by Top Spine Specialist in Pune Dr. Sangram Rajale.

Condition at Discharge:

  1. The patient experienced immediate pain relief after surgery. She was able to walk on day 1, climb stairs on day 2, and was discharged on day 3 after confirming the wound was healthy. 
  2. At the 15-day follow-up, the patient reported feeling as if no surgery had been done.
