History:
A 32-year-old woman who presented to Spine Specialist Doctor in Pune Dr. Sangram Rajale with intermittent low back pain and bilateral lower limb radiculopathy, more severe on the left side. She also experienced tingling and numbness in her right lower limb. Her severe low back pain had been ongoing for 2.5 to 3 years, initially localized to the back but later involving increased radicular pain with accompanying tingling and numbness. She could not stand for more than five minutes and needed to sit down within two minutes, making walking long distances difficult
Examination Findings:
On examination by Orthopedic Spinal Surgeon in Pune Dr. Sangram Rajale of her lumbosacral spine, her Straight Leg Raise (SLR) test was positive at 70° bilaterally, with decreased sensation over the L5-S1 region on the left side. Her MRI revealed spinal canal stenosis at the L5-S1 level due to a diffuse disc bulge, disc extrusion, and thickening of the ligamentum flavum, compressing the thecal sac and bilateral traversing S1 nerve roots. A posterior disc bulge was also noted at L4-L5, indenting the thecal sac.
Outcome:
She underwent L5-S1 transforaminal lumbar interbody fusion on June 10, 2022, performed by Spine Surgery Specialist in Pune, Dr. Sangram Rajale, using NEBULA implants. Two years post-surgery, she is asymptomatic, able to stand and walk for long periods without any discomfort, and fully functional, with no memory of her previous spine issues.
