History :
A 66-year-old gentleman presented to the OPD with a two-year history of low back pain, which has recently been accompanied by bilateral radicular pain in the lower limbs for the past two months. His low back pain exacerbates with activities such as sitting, walking, turning in bed, and bending forward. He also reports tingling and numbness in both lower limbs and can only walk for 15 minutes before needing to sit down. Additionally, he has a history of urinary urgency and retention, along with sleep disturbances due to the pain
MRI :
The patient has multiple large disc protrusions in the lumbar spine at the L3-4, L4-5, and L5-S1 levels, causing significant narrowing (stenosis) of the spinal canal at L3-4 and L4-5, which is compressing the thecal sac and nerve roots. Additionally, there is degenerative arthritis in the facet joints at multiple levels on both sides.
Operation :
On April 26, 2024, posterior spinal fixation was performed from L2 to S1 under general anesthesia. The surgery was conducted by Dr. Sangram Rajale, and involved the placement of spinal implants to stabilize the spine.
Condition On Discharge :
a. The patient is stable and completely pain-free, indicating a successful recovery with no ongoing discomfort or complications from the surgery.
b. Remarkably, the patient was able to sit and walk on the first postoperative day, showing a rapid initial recovery and good mobility immediately following the procedure.
c. Three months after the surgery, the patient has made significant progress and has been able to return to his farming job, demonstrating a full return to his normal daily activities and work responsibilities.
