History :
A 23-year-old man presented with a history of falling from a coconut tree on April 19, 2022. Following this incident, he complained of low back pain with radiating pins and needles sensation in both lower legs, along with more pronounced weakness on the right side compared to the left. These complaints were also accompanied by tingling and numbness. He was initially bedridden for two days before being brought to our hospital on a wheelchair. Since the fall, he had lost control of his bladder and had to crawl to seek assistance nearby. His friends found him and lifted him from the shoulders, but he lacked strength, and his weakness had progressively worsened since the fall.
MRI :
The MRI findings are consistent with the patient’s clinical presentation. The compression burst fracture of the L4 vertebra with superior endplate collapse, along with retropulsion of a bony fragment into the central canal causing central canal stenosis, correlates with the low back pain, radiating pins and needles sensation in both lower legs, weakness predominantly on the right side, and associated tingling and numbness. Additionally, the partial collapse of the L2 vertebra at the superior endplate could contribute to the overall spinal instability and neurological symptoms experienced by the patient.
Operation :

Condition On Discharge :
After two years of follow-up, the patient is now completely pain-free and capable of independently performing all activities. He can bear full weight on both legs and has fully regained strength in his lower limbs. Additionally, his urinary control has returned to normal. Remarkably, he recently achieved the feat of conquering the highest mountain peak in Maharashtra, showcasing his remarkable recovery and restored physical capabilities after the successful spinal surgery and rehabilitation.
