History:
A 58-year-old gentleman presented to Spine Surgery Specialist in Pune Dr. Sangram Rajale with complaints of low back pain and bilateral lower limb pain for the past 2 years. The pain worsened with sitting and bending forward. He reported a history of neurogenic claudication, with symptoms lasting for about 10 minutes, as well as sleep disturbances due to pain. He also had a history of previous spine surgery in 2008. On examination, extension movements were painful and restricted.
Muscle Strength (MRC Scale):
- EHL (Extensor Hallucis Longus): Right 4/5, Left 4/5.
- EDL (Extensor Digitorum Longus): Right 4/5, Left 4/5.

Degenerative changes at the L2-L5 levels with lumbar canal stenosis (LCS) and L4-L5 listhesis
Evidence of previous spine surgery
- Grade I anterior listhesis of L4 over L5
- Right paracentral disc protrusion at L2-L3
- Diffuse disc bulge at L5-S1

CONDITION ON DISCHARGE:
This was the patient’s second spine surgery. Despite being offered surgical intervention by multiple surgeons in the past, he had been hesitant due to concerns about the expected outcomes.
