History:
Previously underwent neck pain treatment through anterior cervical discectomy and fusion also known as ACDF surgery in September 2023 to address severe neck pain. Initially experienced relief from pain, but symptoms began to worsen approximately 20 days post-surgery. Over time, the patient developed increasing weakness in both upper and lower limbs, leading to complete paralysis within eight days. Prior to becoming bedridden, the patient reported experiencing balance issues while walking. Thats when he presented to Spine Surgery Specialist in Pune Dr. Sangram Rajale.
Examination Findings:
- Neck: Painful neck movements.
- Upper Limbs: Complete loss of grip strength, finger abduction, and wrist/elbow/shoulder movements. Deep tendon reflexes were brisk.
- Lower Limbs: Complete loss of muscle strength. Deep tendon reflexes were brisk, and the plantar reflex was extensor.
Diagnosis:
Cervical 5 and 6 infective spondylodiscitis ie infection of the vertebral bone by a bacterial infection of the discs between the vertebrae. The infecting organism was identified as Escherichia coli (E. coli).
Treatment:
- Surgical Intervention: Removal of existing hardware from the neck. Thorough cleaning and debridement of the infected area. Extension of the surgical implant. - Antibiotic Therapy: Long-term antibiotic treatment was initiated based on the identified bacteria and recommendations from an infectious disease specialist.
Outcome:
Immediate Post-Surgery Patient initially showed no movement. Gradual Recovery Over two months, the patient regained lower back pain relief and lower limb strength. Upper limb strength and fine motor skills (e.g., eating, writing) gradually improved. Current Status: After approximately a year, the patient is independent and able to walk without assistance.
